Discharges as follows
- Substantial risk to the resident due to the inability of the home to meet the residents needs and assure the safety and well-being of the other residents in the home.
- Substantial risk, or an occurrence, of self-destructive behavior
- Substantial risk, or an occurrence, of serious physical assault.
- Substantial risk, or an occurrence, destruction of property.
Also
- When a resident has been determined to be at risk pursuant to Sections 11 and 11-A toll-F of Act # 280 of PA 1939, as amended (the Social Welfare Act)
- When a resident has been determined to be at risk due to substantial noncompliance with these licensing rules, which results in the Department taking action to issue a provisional license or to revoke or summarily suspend, or refuse to renew a license, and the resident relocates
- When the facility is no longer able to provide the care that the resident requires.
